Es un linfoma T periférico (LTP) que se caracteriza por enfermedad sistémica, infiltrado polimorfo que afecta a los ganglios , con una proliferación prominente de vénulas endoteliales y células dendríticas.

Representan aproximadamente el 1-2% de todos los linfomas no-Hodgkin y el 15-20% de los LTP. La incidencia es aproximadamente igual en hombres y mujeres. Ocurre más frecuentemente en la edad media de la vida y en los ancianos (media entre 60-65 años).

La mayoría de los pacientes presentan enfermedad avanzada (más de 75% presentan estadio III-IV), con síntomas sistémicos e hipergammaglobulinemia policlonal. En la exploración suelen encontrarse adenopatías, aumento del hígado y el bazo, y frecuentemente «rash» cutáneo; también puede haber edemas, arritmias, ascitis y derrame pleural.

Entre los datos de laboratorio destacan: complejos inmunes circulantes y aglutininas frías con anemia hemolítica. La médula ósea está infiltrada en un elevado número de casos.

La arquitectura del ganglio se altera. Los folículos reactivos con centros germinales desaparecen. El paracortex está infiltrado de forma difusa por una población polimorfa de linfocitos de tamaño pequeño y mediano. Se aprecian vénulas endoteliales abundantes que suelen mostrar arborización.

En el inmunofenotipo se aprecia que los infiltrados están compuestos de células T maduras usualmente CD4 y CD8 mezcladas. Hay proliferación de células dendríticas foliculares (CD21+)

Las anomalías genéticas más frecuentemente encontradas son la trisomía 3, la trisomía 5 y un cromosoma X adicional.

Aunque el tratamiento no resulta óptimo, el más habitual es la quimioterapia tipo  CHOP o CHOP-like. En pacientes seleccionados puede plantearse el trasplante de progenitores hematopoyéticos. Se están comunicando resultados con alemtuzumab y  pralatrexate, lenalidomida, entre otros

Su curso es agresivo y el pronóstico es malo comparado con otros linfomas no hodgkin.
